Statement of Participation - This web site and related services are provided by the MediSys Health Network, Inc. (“MediSys”) to you subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this site will indicate your agreement to be bound by these terms and conditions. If you do not agree to be bound by these terms and conditions, promptly exit this site. The terms and conditions may be modified at any time and such modifications are effective immediately upon their posting. This agreement will be governed by and construed in accordance with the laws of New York.
Health Record Access & Appropriate Use - MediSys MyChart is an Internet application that enables you to have secure, web-based access to portions of your MediSys electronic health record and allows a secure, convenient communications channel with MediSys for non-urgent messages. Use of this service is optional.
You must create a unique identification (ID) code and password that you will use to access health information via MediSys MyChart. This ID and password are unique codes that identify you in the MediSys MyChart computer system. Inquiries and entries made in MediSys MyChart will be logged with these codes. It is extremely important to keep your ID and password completely confidential. If at any time, you feel that the confidentiality of your password has been compromised, you should change it. MediSys takes no responsibility for and disclaims any and all liability for direct, indirect, special, incidental or consequential damages arising from a breach of confidentiality resulting from the loss or sharing of passwords. If it is discovered that you inappropriately shared your password with another person or misused or abused your MediSys MyChart access privileges in any way, your participation in MediSys MyChart may be suspended or discontinued without prior notice.
MediSys MyChart may not be available at all times due to system failures, computer back-up procedures, maintenance or other causes beyond the control of MediSys. If access is restricted for planned maintenance, every effort will be made to communicate downtime to all users in advance. MediSys cannot guarantee access to the MediSys MyChart system and disclaims any and all liability for direct, indirect, special, incidental or consequential damages arising from, or in any way connected with, your utilization of MediSys MyChart or the information contained on the site.
Electronic Communications - MediSys will put forth its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to your inquiry may not be immediately available. You should allow at least three business days for a response. Accordingly, emergency situations requiring immediate attention should never be submitted electronically. Please note that inquiries or messages may be routed to the proper department as necessary for handling, and therefore, MediSys employees, other than your health care provider, may be involved in addressing your inquiry. MediSys MyChart may not be suitable for the transmittal of very sensitive information that you want to discuss directly with your provider and is not a substitute for consultation with your provider. You should never change or stop any course of treatment prescribed by your provider without first consulting him/her.
Privacy and Confidentiality - Your privacy is of the utmost importance to us. MediSys will use your confidential health information to provide you with health care services. Your health information will not be disclosed to any person unless you authorize that person to receive the information. We limit MediSys employee access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption and audit trails are further used to safeguard your information. We identify and track all records released and note the time and date each time you access MediSys MyChart.
As a user of MediSys MyChart, you will be notified via email when there is new health information to be viewed in your chart. Therefore, any person with access to your email will be able to see this notification. This could include your family members, employer or anyone else who can access your email account. Although no private medical information will be sent, you should take this into consideration when providing an email address or using MediSys MyChart.
Information Accuracy and Integrity - While MediSys attempts to ensure the integrity of the site, it makes no guarantees as to the correctness and accuracy of the site. Information in MediSys MyChart is provided “as is” without warranty of any kind and is intended to support your relationship with your current or future provider. Reliance on the material in MediSys MyChart is at your own risk. Health information provided is not a replacement for proper medical attention. It is possible that information could contain typographical errors, inaccuracies or other errors. Information contained on the site may be changed or updated without notice. MediSys MyChart is the property of MediSys and the information displayed may not be your complete medical record. Contact MediSys directly for a copy of your health record or in regard to discrepancies with the health information listed in your MediSys MyChart.
Minor Eligibility - An individual must be 18 years or older to request a personal account. Parents or guardians of minor children may request access to the minor child’s account by following the terms outlined in the “Proxy Access” section of this Terms and Conditions agreement. Minor children between the ages of 12 and 17 years (inclusive) will not be eligible to access MediSys MyChart services. Under state and federal laws, there are certain types of medical information that the parent or guardian of a minor patient age 12 years or older may not view without the consent of the minor patient. Because of these requirements, we offer limited access to parents of minor patients in this age category.
Proxy Access - A parent or legal guardian of a minor child up to the child’s 18th birthday or individuals 18 years and older may request proxy access to another individual’s MediSys MyChart account by completing the Proxy Access portion of the MediSys MyChart Consent Form and submitting it to MediSys. Such access will only be granted to parties with parental rights or legal guardianship over the MediSys account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to the account holder’s health information. Only one person will be granted proxy access to a MediSys MyChart account. A MediSys MyChart account will be activated for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must inform MediSys. MediSys reserves the right to revoke proxy access at any time for any reason.
Surveys - You may be asked to complete surveys or questionnaires from time to time to help optimize your health care and improve MediSys MyChart.
